Options/Solutions Program

The Options & Solutions program helps seniors and their families sort through information and services that are available, finding solutions to their individual situation.  For seniors, it's about balancing their desire for independence with support for any needs they have.  For families, it's about peace of mind and a sense of security.

Do you have any of these concerns about your family, friends or neighbors?

• Are they having trouble caring for a loved one?
• Are they safe in their home? Are they eating well, becoming forgetful, falling, taking their medications, driving safely, etc.
• Are they able to manage their household? Are they keeping their house clean, doing laundry, grocery shopping and cooking, paying their bills on time, etc.
• Have you noticed a neglect in their personal hygiene? Are their clothes unkempt, are they not bathing well, keeping up with their dental hygiene, nail care, etc.
• Is their situation impacting your work or personal life? Are you missing work, is there stress on your family, are you and your siblings having disagreements, etc.
• Are you having trouble approaching your loved one with any of these concerns?

If you answered yes to the questions above, Valley VNA can help.  Through our Options & Solutions program you will:

• Work with someone you can trust. Valley VNA has been assisting families in the community for 100 years.
• Talk with an experienced RN who will listen to you. Valley VNA's RN's have extensive training and experience related to senior issues.
• Set up a FREE consultation (1-2 hour visit). Our RN's will arrange a personal meeting - in your home, the hospital or nursing home. They will assess the senior's needs, current living arrangements, and existing support resources.
• Receive recommendations on options available. Based on the consultation, our RNs will provide written recommendations and options for services available in the senior's community - home-delivered meals, transportation services, home care, assisted living, etc.
• Learn about additional services from Valley VNA Senior Services. Valley VNA is able to provide many services (Fee For Services) including:
  • Advanced assessments - Cognitive Screen, Depression Screen, Safety Assessment.
  • In-Home Care - our RNs can develop and supervise an individual care plan that will be provided by specially trained staff.
  • Assisted Living - Valley VNA offers several levels of care from independent to late-stage dementia care.
